A Case of Esophageal Penetration of a Fish Bone into the Left Atrium Rescued by Urgent Surgery

<b>INTRODUCTION:</b> Esophageal penetration due to accidental foreign body ingestion is relatively rare but has a poor prognosis. Penetration into the left atrium is extremely rare, and only a few cases have been reported. A case of foreign body penetration into the left atrium is reported.<b>CASE PRESENTATION:</b> The patient was a 75-year-old woman who was transferred to our hospital for surgery due to fish bone perforation from the lower esophagus into the left atrium on CT. Urgent surgery was performed with the diagnosis of sepsis due to a mediastinal abscess and esophageal penetration into the left atrium caused by accidental fish bone ingestion. At surgery, only the fistula of the penetration wound in the esophagus and the left atrium could be detected, but not the fish bone. On CT after surgery, a folded fish bone was seen at the pericardium close to the left atrium. It was considered unlikely that the heart would again be perforated due to the fish bone’s length, and it was decided to follow up with continued antimicrobial therapy.<b>CONCLUSIONS:</b> A rare, successfully rescued case of esophageal penetration of an accidentally ingested fish bone into the left atrium by urgent surgery, with collaboration between gastrointestinal and cardiovascular surgeons, is reported.

骨盤骨折を伴う会陰部の複雑性外傷を契機に広範な壊死性軟部組織感染症を経験した．症例は59歳，男性．作業中にクレーン車に骨盤部を挟まれ，近医に緊急搬送された．恥坐骨骨折と右臀部，会陰部の挫滅創の診断で処置され入院となった．受傷後4日目に高熱と会陰部創からの悪臭を伴う膿性排液が認められ，背部と大腿部に広範の発赤と腫脹をきたしていたことより，当科に緊急搬送となった．当院での精査の結果，会陰部外傷による壊死性軟部組織感染症の診断で緊急開創ドレナージ術，人工肛門造設術を施行した．約2週間に及ぶICU管理と1カ月を超える感染症治療を行った．計4回のdebridementを行い，分層植皮にて治癒を得ることができた．術後1年で人工肛門を閉鎖し，就業可能となった．臀部の外傷，特に肛門に近接している骨盤骨折を伴う会陰部の開放創は適切な管理が極めて重要である．

Summary
Spindle epithelial tumor with thymic-like elements (SETTLE) is an extremely rare tumor that occurs primarily in the thyroid gland. Histologically, SETTLE is characterized by the presence of spindle-shaped epithelial cells and glandular structures. However, it is known that diagnosis via fine-needle aspiration cytology can be challenging. SETTLE predominantly occurs in younger individuals and has a less favorable prognosis compared to differentiated thyroid carcinoma. Therefore, ensuring accurate diagnosis and appropriate treatment is crucial. We encountered a case of spindle epithelial tumor with thymus-like differentiation in a 10-year-old patient for whom the preoperative diagnosis was successfully established through fine-needle aspiration cytology, which facilitated appropriate surgical resection. Comprehensive histopathological examination and immunohistochemical analysis are essential to ensure appropriate management and surveillance of SETTLE.
Learning points
A rare thyroid tumor, spindle epithelial tumor with thymic-like elements (SETTLE), was diagnosed preoperatively and treated surgically.
SETTLE presents with characteristic histological features that must be recognized for accurate diagnosis. In addition, diagnosis through cytology is often challenging.
The primary treatment for SETTLE is surgical intervention as radiotherapy and pharmacological treatments are generally not expected to be highly effective.

〔要旨〕Acute care surgery（ACS）を実践した10年間を振り返り，地方都市に求められるACSのあり方を検討した。当院は2012年に救命救急センターが稼働し，外科救急の診療体制に大きな変化が求められた。地域レベルでは，既存の救急体制の維持を重視し重傷例や外傷症例を当院に集約するシステムをとった。施設レベルでは，外傷診療を円滑に行うため既存の外科医によるACSチームの結成と積極的外傷診療の介入を行った。また，チーム維持のためにオンコール体制の充実と周術期管理の完全分業化を実践した。個人レベルでは，手術スキルやモチベーション維持のためスペシャリティをもち日常外科業務を行った。また，ACS育成のため若手外科医，研修医の診療介入を後押しできるように努めた。このように地方都市におけるACSのあり方は，既存の救急システムを維持し，既存の外科医が施設に即したシステムをつくることによって成立すると考える。
