    We conducted a questionnaire survey on basic clinical-skills training and resuscitation training for newly employed residents. We compared the responses between the residents who had experienced simulation-based training (simulation group, n = 33) and those who had experienced video-assisted lectures (video group, n = 26). Across the eight procedures, the percentage of respondents who was "useful for subsequent clinical practice" was significantly higher in the simulation group than in the video group (91% vs. 75%, p = 0.001). We need to consider factors such as the frequency and difficulty of the procedures, the content of available teaching materials, and the curriculum in each medical school. In addition, we have to seek out practical training content with better teaching efficiency and better learning efficiency by effectively incorporating practical skills.

      Acute encephalitis with refractory, repetitive partial seizures (AERRPS) are extremely intractable encephalitides that require intensive care, including long-term mechanical ventilation management. Continuous intravenous administration of barbiturate is commonly performed for seizure suppression ; however, it may have serious complications. Here, we present a 6-year-old boy with AERRPS. Generalized tonic seizures followed by focal seizures frequently occurred after high-grade fever. Seizures were refractory to conventional treatment, and ventilatory management and brain normothermia therapy were then performed in the intensive care unit. Following diagnosis of AERRPS, administration of continuous thiamylal infusion resulted in disappearance of seizures ; however, it induced organ damage. Thus, administration of very-high-dose phenobarbital (PB) (maximum blood concentration is 243μg/mL) instead of thiamylal was selected. As seizures recurred when the dose of PB was reduced, concomitant treatment with a ketogenic diet was performed, and the seizures disappeared and the electroencephalogram improved promptly. To withdraw PB, perampanel (PER) was selected. PER administration induced a reduction in PB and withdrawal of mechanical ventilation on day 49. Subsequently, the patient was adjusted for antiepileptic drugs and the seizures became weekly short focal seizures, and was discharged on foot. In conclusion, we experienced a case of AERRPS that could be managed with very-high-dose PB therapy, which had the same efficacy as the continuous intravenous high-dose barbiturate infusion. Moreover, PER is effective for withdrawal from acute treatment, and active use of PER for younger AERRPS is considered to be one of the options.
