本研究では、成人を迎えたCHD患者の妊娠出産/親になることへの認識やその経験を通して起こった身体的/精神的変化を明らかにし、妊娠出産/親になるために必要な能力を身につけるための支援の検討を目的としている。研究は3段階で構成されており、これらの結果から妊娠出産/親になるために必要な社会的支援/精神的支援を検討していく予定である。
研究1として「妊娠出産/子育てに関する現状の把握」のために国内外の文献検討を行った。研究2では「成人を迎えたCHD患者の妊娠出産/親になることへの認識の把握」のために、成人を迎えたCHD診療専門医療施設にて定期的または不定期的に外来受診を行っている18歳以上のCHD患者へアンケート調査を行う予定である。しかし、この2年間COVID－19の影響で医療機関での調査が困難であったため、文献検討結果と以前他の研究者と共同で実施した「成人CHD女性の結婚・妊娠・出産への思いについて」のアンケート結果の一部を使用して『成人先天性心疾患女性のリプロダクティブ・ヘルス向上に向けた妊娠出産に関する予備調査』としてまとめ、妊娠出産経験のない女性のみの妊娠出産に対する思いを明らかにしている。しかしながら、CHD男性も含めて親になることについての認識（思い）の把握ができていないため、この内容の調査を行うために調査方法を検討したのち、対象施設へ研究協力の依頼を再度行っていかなければいけない状況である。
研究3での「妊娠出産/親になった経験が与える身体的/精神的変化の把握」のためのインタビュー調査については、対象者への依頼をすすめている。
研究2、研究3の実施順序については、今後支援内容の検討を行う上で実施順序が結果に大きな影響を与えないため、感染予防対策が行いやすく、実施可能な調査から実施していくなどの研究スケジュールの調整を行っている状態である。 -

The aim of this study was to create the database of the clinical backgrounds of whole life, health problems of body and mind in CHD patients. As the education of patients and their family, we held seminars and lectures. We also aimed to establish an interactive medical information system that enabled lifelong medical management of patients at related medical institutions. We collected clinical information of 600 patients from childhood and perinatal data of 150 patients using REDCap system. The survey items were demographic data, clinical history, present status, medications, and laboratory data (blood examination, chest XP, echocardiography, and ECG). Those data were recorded on the REDCap system (Research Electronic Data Capture, Vanderbilt University) and will be analyzed. From now on, we will collect the data from Fukuoka municipal children’s hospital and connect the data with ours. Those connected data becomes very efficient for the follow-up of ACHD patients in future.
