    Various conditions, including schistosomiasis, have been reported as potential causes of false-positive hepatitis C virus (HCV) antibody test results; however, there have been few reports of false-positive results associated with cestodiasis or reports of HCV antibody test results turning negative following deworming. In this report, we describe a woman in her 30 s with tapeworm infection who showed a false-positive HCV antibody test result that turned negative after deworming. False-positive HCV antibody results associated with cestodiasis may represent a particular diagnostic challenge in developing countries, where confirmatory HCV genetic testing is less readily available.

    Human toxocariasis is a parasitic infection caused by Toxocara species and is important in the differential diagnosis of patients presenting with unexplained eosinophilia. A case of toxocariasis with an eosinophilic pleural effusion is presented. A 67-year-old Japanese man presented with progressive dyspnea. Laboratory investigations showed marked peripheral eosinophilia and a markedly elevated serum immunoglobulin E level. Chest imaging demonstrated bilateral ground-glass opacities and a left-sided pleural effusion. Analysis of the pleural fluid showed eosinophil predominance without evidence of malignancy or bacterial infection. Extensive diagnostic evaluations, including autoimmune serology, bone marrow examination, and genetic analyses for myeloid neoplasms with eosinophilia, yielded negative results. Serological test using an enzyme-linked immunosorbent assay demonstrated elevated anti-Toxocara antibody level, which were subsequently confirmed by Western blot analysis, leading to a diagnosis of human toxocariasis presenting with visceral larva migrans. The patient had no clear history of animal contact or ingestion of raw meat or liver. Treatment with oral albendazole for four weeks resulted in marked clinical improvement, with resolution of the pleural effusion and pulmonary infiltrates, normalization of peripheral eosinophil counts, and a significant decrease in anti-Toxocara antibody level. This case emphasizes the importance of considering toxocariasis in the differential diagnosis of an eosinophilic pleural effusion and pulmonary infiltrates, even in the absence of identifiable exposure risks. Early serological testing can facilitate prompt diagnosis and appropriate anthelmintic therapy, potentially preventing unnecessary invasive procedures and delayed treatment.

    Zoonotic onchocerciasis is a rare human infection caused by Onchocerca species that normally parasitize non-human mammals. In Japan, all previously reported human cases have been attributed to Onchocerca japonica and have presented as localized, non-migratory subcutaneous nodules. Here, we report the first human infection caused by Onchocerca takaokai Uni et al., 2015. A 24-year-old male presented with linear migratory erythema on the forearm, clinically consistent with creeping eruption. Histopathological examination revealed an adult female filarial nematode with polymyarian-coelomyarian musculature, without internal cuticular ridges in the lateral cords, and lacking transverse ridges on the cuticle. Molecular analyses of the mitochondrial 12S rRNA and cytochrome c oxidase subunit I (cox1) genes confirmed that the parasite was O. takaokai, a parasite of wild boars in Japan. This case demonstrates a clinical presentation distinct from that of O. japonica and suggests that O. takaokai should be considered in the differential diagnosis of creeping eruption in endemic areas.

    Seroprevalence of Paragonimus infection in sika deer was evaluated to assess the risk of paragonimiasis through venison consumption. Total 1,546 blood samples collected from sika deer from February 2013 to March 2020 in western Japan were tested by pre-adsorbed ELISA, and 1.4 % of them were positive for anti-Paragonimus antibodies. Our results suggested that venison can be a risk of the infection in the endemic areas of paragonimiasis, although the infection risk of venison would be lower than that of wild boar meat considering the positive rate in wild boars in these areas (9.1-43.0%). In addition, the leg of Japanese freshwater crab, Geothelphusa dehaani, was found in the rumen of sika deer, suggesting sika deer got Paragonimus infection by the ingestion of Japanese freshwater crab, the first intermediate host.
