Objectives: Anti-resorptive agents (ARA), such as bisphosphonates and denosumab, are first-line treatments for bone metastases from solid tumors and multiple myeloma. Although these agents effectively inhibit bone resorption, relieve pain associated with bone metastases, and inhibit the growth of bone metastases in malignant diseases, the risk of medication-related osteonecrosis of the jaw (MRONJ) remains a significant concern. In this study, we retrospectively examined patients who received high-dose ARA at our hospital after their initial visit to our department, focusing on their oral condition at the time of the initial visit and at the start of high-dose ARA treatment. Methods: Between 2013 and 2023, 91 patients who received high-dose ARA at our hospital after their initial visit to our department with at least 3 months of follow-up were retrospectively investigated from their medical records. We examined various factors that may increase the risk of MRONJ, but we focused on dental conditions at high-dose ARA administration. Results: Univariate Cox regression analysis showed that sex, steroid use, tooth indicated for extraction at initial visit and tooth indicated for extraction at high-dose ARA administration were significantly associated with MRONJ development. Furthermore, multivariate Cox regression analysis showed that steroid use and tooth indicated for extraction at high-dose ARA administration were significantly associated with MRONJ development. (p < 0.05). Conclusion: A pre-treatment dental examination and timely extraction of the indicated tooth prior to high-dose ARA administration may reduce the risk of MRONJ. These findings highlight the importance of early dental intervention in patients receiving high-dose ARA treatment.

Epithelioid schwannoma in the buccal region: A case report and review of literature 査読あり

Schwannomas are common, benign peripheral nerve sheath tumors that arise from Schwann cells. Several rare morphological variants include ancient, cellular, plexiform, epithelioid, and microcystic/reticular. The epithelioid variant of schwannoma is scarce, with only a few relevant cases reported in the oral and maxillofacial regions. Herein, we report a case of epithelioid schwannoma in the buccal region. A 61-year-old woman was referred to our department from a medical clinic because of a slow-growing nodule in the buccal region, noticed 3 years before presentation. Clinical examination revealed a nodular mass measuring 20 × 20 mm. Computed tomography (CT) and magnetic resonance imaging (MRI) showed an oval lesion with a clear border on the left cheek. The outer margin of the mass exhibited opacities that appeared as multiple microcalcifications. MRI revealed a well-defined mass similar in size to that observed on CT, with low signal intensity on T1-weighted images and mildly high signal intensity on T2-weighted images. The clinical differential diagnosis includes benign tumors such as pleomorphic adenoma. The tumor was surgically excised under general anesthesia. Histopathological examination revealed multilobulated epithelioid cells arranged in nests. The tumor cells showed eosinophilic cytoplasm and uniformly round nuclei. The tumor was diagnosed as an epithelioid schwannoma. Postoperative follow-up was uneventful, and no evidence of recurrence was observed 2 years postoperatively.

Myxolipoma in the buccal region: A case report and review of literature 査読あり

Lipomas are benign mesenchymal tumors with a relatively high incidence rate. However, myxolipomas, a histological variant characterized by mature adipose tissue and abundant myxoid substances, are rare, especially in the oral cavity. This case report focuses on a 90-year-old-female patient, the oldest reported with oral myxolipoma, who presented with the chief complaint of a painless mass in her right buccal region that had persisted for several years. The mass (approximately 25 mm in diameter) was palpable, soft, elastic, and covered with normal mucosa. In general, the Hounsfield unit (HU) of lipomas is approximately –100 units on computed tomography, and lipomas show high signal intensity on both T1- and T2-weighted magnetic resonance imaging. In contrast, in this case, the HU was approximately 50 units, which is slightly lower than that of the common muscle, similar to a previous report on myxomas. Unlike common lipomas, the present case exhibited low and high signal intensities on T1- and T2-weighted images, respectively. The tumor was surgically excised under local anesthesia and histopathological examination revealed a well-defined lobulated mass surrounded by a thin fibrous capsule. The tumor cells exhibited a solid proliferation of mature adipocytes, which were replaced by abundant myxoid substances. Based on these findings, the patient was diagnosed with a myxolipoma. Myxolipoma should be considered as a differential diagnosis when myxomas are suspected on preoperative imaging studies. The postoperative follow-up was uneventful, and no evidence of recurrence was observed 2 years postoperatively.

Chondrolipoma arising in the tongue and related literature review: A report of two cases 査読あり

Chondrolipomas are rare benign tumors composed of mature adipocytes, a metaplastic chondromatrix, and cartilaginous chondrocytes. Herein, we present two cases of chondrolipoma of the tongue. The first case involved a 62-year-old woman who had a painless mass on the border of her tongue for four years; the second patient was a 74-year-old woman with a painless mass on the dorsal aspect of her tongue that had persisted for approximately six decades. Both cases were clinically diagnosed as benign tumors and the patients underwent surgical excision under general anesthesia. No local recurrence was observed during their 5-year postoperative follow-up periods. We further investigated 19 cases of chondrolipoma arising in the lip and oral cavity, including 17 previously reported cases and the two presented cases. The cartilage found in these tumors likely represents metaplastic changes or hamartomatous proliferation, attributed to the multipotentiality of mesenchymal stem cells.

Introduction: The extent of surgical resection for tongue tumors is determined by tumor size, potentially affecting oral function and quality of life (QoL). However, the relationship between oral dysfunction and QoL decline due to glossectomy extent remains unexplored. Therefore, these correlations and their predictive value for postoperative QoL decline were elucidated. Methods: Patients treated for tongue cancer at our hospital between 2018 and 2022 were categorized by partial, hemi, or subtotal/total glossectomy. Assessments included swallowing function (RSST), articulation (Oral Diadochokinesis (ODK)), mastication, tongue pressure, and oral moisture. QoL was measured using the Oral Health Impact Profile-14 (OHIP-14). Differences within parameters were assessed using Kruskal–Wallis tests, and between-group comparisons via Mann–Whitney U tests. Spearman's correlation analysis examined parameter relationship. Results: 35 patients were evaluated. Significant differences were found in ODK [ta] (p = 0.015), [ka] (p = 0.0006), tongue pressure (p = 0.0001), moisture levels (p = 0.031), OHIP-14 domains: physical disability (p = 0.014) and social disability (p = 0.046). ODK [ta] (PG: 5.95, HG: 5.38, TG: 4.03 times), [ka] (PG: 5.56, HG: 4.78, TG: 3.23 times), and tongue pressure (PG: 32.9, HG: 21.2, TG: 10.3 mmHg) decreased with glossectomy extent, while physical (PG: 0.27, HG: 2.38, TG: 2.00) and social disability (PG: 0.18, HG: 0.94, TG: 1.43) worsened. A significant negative correlation was observed between tongue pressure and social disability (p = 0.013, r = -0.36). Conclusion: Expanding resection significantly impacted postoperative oral function and QoL. Tongue pressure assessment may predict long-term social disability in patient QoL.

骨芽細胞由来液性因子によるオーダーメイド骨再生医療のための基盤形成

骨芽細胞系細胞に圧縮刺激を加えて得られた培養上清(MsCM-OB)の骨形成・骨再生における作用を検討し、骨再生医療への応用を目指す。申請者はこれまでに予備的研究からMsCM-OBに含まれる液性因子が骨形成を促進する可能性を見出している。そこで本研究では、骨折モデルマウスならびに頭蓋骨欠損モデルマウスにMsCM-OBを局所投与し、骨折治癒や骨欠損修復に対するMsCM-OBの作用を検証する。そして将来的に、例えば“患者から樹立されたiPS細胞から骨芽細胞を分化させ、機械的圧力を加えて培養上清(MsCM-OB)を回収し、骨欠損部に局所投与する”といった患者由来の液性因子を用いた副作用の少ないオーダーメイドの顎骨再生医療が実現するための基盤を形成する。
