<p>Comprehensive pharmacology education in nursing based on the “Patient-oriented Pharmacology” is effective against the improvement of quality of pharmacotherapy and patient satisfaction. Two active learning programs of practical pharmacotherapy for nursing students have been performed in School of Nursing, University of Miyazaki; (1) pharmacotherapy role-play for interprofessional education (IPE) and (2) practical excise for Kampo medicine. Pharmacotherapy role-play for IPE was performed as joint lecture both medical students and nursing students. This pharmacotherapy role-play is named Case & Communication based approach (C&C approach), since it is studied through communication between physicians, nurses and patients based on cases presented beforehand. In the practical excise for Kampo medicine, nursing students studied Kampo medicines and tried to taste 9 frequently used Kampo medicines. These active-learning programs in nursing pharmacology education may be effective for better understanding of pharmacotherapy and patient’s feeling, and improvement of students’ motivation as a nurse.</p>

＜文献概要＞はじめに 看護における薬理学教育は、看護職の専門化・多様化・高度化に伴い、より重視される傾向にある。与薬の実践者である看護師には、薬の専門家である薬剤師とともに"患者を守る最後の砦"として、薬物治療に関して高度で幅広い知識が求められている。"与薬は患者への介入を伴う"ものであり、その安全性を担保するために、十分な薬理学教育が必要となる。大学病院などでの看護師への現任教育では、薬剤部や診療科と連携しながら薬物治療に関する教育が展開されている。助産師教育に関しては、大学院修士課程への移行や助産実践能力習熟段階(CLoCMiP)において、より高度な看護実践者として妊娠授乳期の薬物治療や薬剤管理に関する専門的知識の習得が望まれている。また、「専門看護師教育」「認定看護師教育」「特定行為に係る看護師の研修制度」において、臨床薬理学は必修となっており、それらの臨床薬理学教育に立脚した「薬物療法看護」の充実が、さまざまな看護領域において求められている。卒後教育や継続教育において薬物治療に関する教育が重視されている一方で、看護師養成課程における薬理学教育は十分と言えない状況であったが、第5次指定規則改正において、「解剖生理学や薬理学等を充実させ、臨床判断能力の基盤を強化する」ために、「人体の構造と機能」および「疾病の成り立ちと回復の促進」について1単位増加が示された。また、2017年に策定された「看護学教育モデル・コア・カリキュラム」においても、薬理学教育の充実が求められている。本稿では、看護実践に必要な薬理学知識を学ぶための教育について紹介したい。

大腸がんは日本人が罹患するがんの第1位であり、高齢での発症が増えている。本申請では、欧米と治療方法の相違がある直腸がんの患者を対象として、実装可能な高齢者総合機能評価 （CGA）活用型支援モデルを構築する。CGAに基づく介入は、死亡率の低下、せん妄を含む術後合併症や有害事象の軽減、QOLの向上において有効性が支持されており、身体機能・併存症・認知機能・精神機能・社会的環境・栄養・老年症候群などを総合的に評価し、個々に適切な介入を行うことが推奨されている。高齢がん患者数が急増していく中、本申請の成果により、高齢がん患者全体のQOL向上が期待できる。 -

がんサバイバーの家族は、環境やライフスタイル、遺伝的リスク要因などを共有しているため、がん罹患率は高くなる傾向にあり、予防は重要となる。本申請では、米国のデータ利用により、がん検診受信の影響要因をがんの部位別に日米比較する。また、日本人がんサバイバーの家族に調査して、サバイバーのがん再発の恐れは当人だけではなく、家族のがん検診受診行動と関連しているというACSデータの研究成果と比較する。これらを基に、がんサバイバーの家族のがん予防行動支援モデルの構築を目指す。本申請は今まで改善できなかった日本人のがん検診受診率向上に寄与できる可能性を秘めており、挑戦的研究として意義がある。
