    On 1 January 2024, a large earthquake occurred in Japan’s Noto region. Many buildings collapsed as a result of violent shaking. Electricity and water supplies were cut off, and communications were disrupted. On 5 January, four days after the earthquake, we visited Noto and conducted disaster-relief activities. This report integrates and discusses the results of the site visits, information broadcasts by public institutions, and previous research. Evacuation centers lacked water and proper sanitation, leading to health issues, including infectious diseases. Disaster Medical Assistance Teams (DMAT) were delayed in implementing infection control measures. Isolated evacuation centers faced communication and supply challenges. Infrastructure restoration, power supply, and toilet facilities at evacuation centers were delayed because of geographical challenges. It is important to have a team that can determine and carry out the necessary activities on site, even without instructions from the DMAT. It is believed to be effective to decide in advance how volunteer teams and the private sector will conduct their activities, assuming that they will be unable to contact public institutions during a disaster. In large-scale disasters, evacuees must operate evacuation centers autonomously. To achieve this, it is necessary for residents to regularly come together as a community. Systematically recording and accumulating these experiences will contribute to improved disaster prevention and mitigation planning. We hope that the experiences we obtained through the abovementioned disaster will be useful for preparing for future disasters.

    Background: The impact of chronic kidney disease (CKD) on healthy life expectancy and healthcare costs requires research. This study examined associations between CKD and healthy life expectancy, and its economic burden. Methods: This study of community-dwelling adults residing in Hakui City, Ishikawa Prefecture, Japan used data from the National Health Insurance database between 2012 and 2022. Participants were grouped by baseline estimated glomerular filtration rate (eGFR) (< 45, ≥ 45 to < 60, ≥60 to < 75, ≥75 to < 90, and ≥ 90 mL/min/1.73 m²). The primary endpoint was a composite of becoming a care level ≥ 2 or death. Multivariable Cox proportional hazards models were used to calculate the risk regarding time to the primary endpoint. Secondary endpoints were the annual medical and long-term care costs. Results: The 5,592 participants had a mean follow-up of 6.4 years. The hazard ratio was 1.86 (95% confidence interval [CI]: 1.35 to 2.55) for the eGFR < 45 group and 1.60 (95% CI: 1.13 to 2.25) for the eGFR ≥ 90 group, both compared with the eGFR ≥ 60 to < 75 group. Both annual costs were significantly higher in the lower eGFR groups than in the higher eGFR groups. For the eGFR < 45 group, the median medical care cost was 0.38 million yen/year in all participants and the median long-term care cost was 0.40 million yen/year in primary endpoint achievers. A lower eGFR was correlated with longer unhealthy years of life. Conclusions: Higher and lower eGFRs were associated with increased risks of reduced healthy life expectancy. A lower eGFR was associated with higher medical and long-term care costs.

    医療的ケア児・者などの災害時要支援者が、担当の専門職と一緒に自分の情報や必要な備えをスマートフォン(スマホ)のアプリケーションソフト(アプリ)に入力しておき、災害時に開示することで迅速な治療、看護や搬送に役立つアプリ(K-DiPS Solo)を開発した。さらに、K-DiPS Soloの情報を、通信回線を介してサーバーに接続し集約することで、地図上に居場所、状態像などを可視化、必要な支援、備蓄を自動算出できるアプリ(K-DiPS Online)を開発している。このK-DiPSの機能として医療的ケア児の医療機器やバッテリー医療衛生材料を画像認識しIoTで管理できるシステムの開発を目指しているが、その過程で、K-DiPS Soloの情報を用いた訪問看護ステーション版の事業継続計画(BCP)を立案できるシステムの開発の優先順位が高まった。そのために、2020年度に高知県訪問看護連絡協議会と訪問看護ステーション版BCP案を作成した。2022年度に、K-DiPSの機能ととしてプロトタイプを作成し実証予定である。

    This study examined the characteristics of care plans for dementia patients living at home by care managers and devised care plans making the most of basic qualifications. A questionnaire survey on care plans was conducted, involving 1,104 care managers. More than 50% of all respondents perceived difficulty in collaborating with legal institutions, regardless of their basic qualifications. Furthermore, more than 60% perceived difficulty due to patients’ refusal to receive medical consultation, and insufficient collaboration with legal institutions was associated with such a sense of difficulty. Based on these findings, strategies to promote collaboration were devised with 5 care managers who had basic qualifications such as care worker or nurse.

    We conducted a survey on disaster prevention for children with developmental disorders living in the community. Based on the survey results, an evacuation behavior simulation was conducted using the Geographic Information System (GIS). Then, we developed application software that allows children with developmental disasters and caregivers to prepare for their own disaster together with their medical professionals. This app has been developed by us for a long time, and this app has been developed so that it can be used by all patients who live in the area as well as developmental disorders. Using the app, we conducted a demonstration test in two local governments (Tsukitsuki, Kagoshima and Uchinada, Ishikawa). As a result, some challenges became clear. In the demonstration test, NTT Docomo lent an iPad. I was allowed to use the LTE line free of charge. The app aims to fix the issues revealed in the study and put it into practical use by the end of 2020.
