This study aimed to examine whether preterm birth is associated with thinness and differences in body mass index (BMI) trajectories at ages 3-6 years. This cohort study used data from the Japan Environment and Children's Study, a nationwide birth cohort. Among 100,121 live births, 93,406 children were included. Preterm birth was defined as < 37 weeks' gestation. Thinness was defined as BMI-for-age < - 2 standard deviations according to World Health Organization criteria. Multivariable logistic regression was used to examine associations between preterm birth and thinness at each age. Longitudinal changes in BMI were analysed using linear mixed-effects models. Preterm birth was associated with higher odds of thinness at most ages: adjusted odds ratios were 1.35 (95% confidence interval [CI], 0.94-1.93) at age 3 years, 1.43 (95% CI, 1.02-1.99) at 4 years, 1.70 (95% CI, 1.29-2.25) at 5 years, and 1.52 (95% CI, 1.20-1.92) at 6 years. Findings were consistent with sensitivity analyses using an alternative definition of thinness. In the longitudinal analysis, mean BMI was consistently lower among preterm-born children, with mean BMI differences of 0.11-0.19 kg/m2 across ages 3-6 years, and no evidence of convergence over time. CONCLUSION: Preterm birth was associated with lower BMI trajectories and higher odds of thinness during early childhood. These findings suggest that incorporating gestational age into growth assessment and monitoring body mass, in addition to linear growth, may help identify children with lower BMI trajectories. WHAT IS KNOWN: • Children born preterm have heterogeneous postnatal growth trajectories, with previous studies reporting both increased risk of obesity and lower body mass during childhood. • Large-scale longitudinal evidence on BMI and thinness during early childhood among children born preterm remains limited. WHAT IS NEW: • In this nationwide birth cohort, preterm birth was associated with higher odds of thinness at ages 4-6 years. • Children born preterm also had consistently lower BMI at ages 3-6 years, with no evidence of convergence with term-born children.

We previously reported inherited RORγT deficiency in seven patients from three ancestries (Chilean, Palestinian, Saudi Arabian) with mycobacterial disease and chronic mucocutaneous candidiasis (CMC). We report here five additional patients from different ancestries (Afghan, Indian, Iranian, Japanese, Sri Lankan), each homozygous for a new loss-of-function RORC variant. All but one patient — the exception receiving early prophylaxis — developed mycobacterial disease due to a near-complete depletion of innate-like adaptive T cells, including MAIT and iNKT cells, low counts of adaptive TH1* and CD8+ T cells, and impaired Mycobacterium-induced IFN-γ production by the remaining cells of these subsets, NK cells, conventional CD4+ T, Vδ1, and Vδ2 γδT cells. Most patients also displayed CMC due to their low counts of TH17 and TH1* cells. One patient died from disseminated Bacille Calmette-Guérin (BCG) vaccine infection, but, unexpectedly, all the other patients are still alive and clinically stable at ages of two to 20 years. RORγT is essential for protective immunity against mycobacteria and Candida in humans.

    Background:
Osteogenesis imperfecta (OI) is a congenital bone fragility disorder, in which 90% of patients have abnormalities in type 1 collagen. Cardiovascular complications, including valvular heart disease and vascular aneurysms, are reportedly comorbid with this disease.
    Methods:
Forty-three adult Japanese patients clinically diagnosed with OI were recruited. Participants underwent echocardiography, brain magnetic resonance imaging (MRI), and magnetic resonance angiography. We compared the prevalence of cardiovascular comorbidities with that of a healthy Japanese population using direct age-standardization and a one-sample binomial test.
    Results:
The median age was 42.5 (IQR 37.4–53.5) years. OI severity was mild in 11, moderate in 11, and severe in 21 patients. Two patients had ⩾ moderate aortic regurgitation (moderate OI: 1; severe OI: 1), one patient had severe mitral regurgitation (moderate OI: 1), and another had previously received valvuloplasty for mitral regurgitation (mild OI: 1). The prevalence of valvular heart disease (9.3%) was significantly higher than that of the healthy Japanese population (0.8%) (p < 0.001). Five patients (11.6%) had unruptured intracranial aneurysms (UIAs) (mild OI: 2; severe OI: 3), and the prevalence was significantly higher than that of the healthy Japanese population (2.5%) (p < 0.005). Notably, two patients presented with both valvular heart disease and a UIA. Additionally, 27 patients (62.8%) had tortuosity in the intracranial arteries, and seven patients (16.3%) had significant cortical atrophy.
    Conclusion:
This study revealed an increased prevalence of valvular heart disease, UIAs, and possibly intracranial arterial tortuosity and cortical atrophy among adults with clinically diagnosed OI. Routine echocardiography, and brain MRI and MRA are recommended regardless of disease severity.

INTRODUCTION: The prognosis of myeloid leukaemia associated with Down syndrome (ML-DS) is excellent with reduced-intensity chemotherapy; however, that of refractory or relapsed ML-DS remains dismal. These contrasting outcomes highlight the importance of risk-stratified therapy, based on reliable prognostic factors, to prevent treatment-related mortality and relapse. In addition, novel therapeutic strategies capable of overcoming the poor prognosis of high-risk patients are eagerly anticipated. Based on the results of molecular analyses, in vitro functional assays and anecdotal case reports, azacitidine (AZA), a hypomethylating agent, is a promising candidate for novel therapeutic strategies for high-risk ML-DS. METHODS AND ANALYSIS: In this multicentre, single-arm phase II clinical trial, the Japan Children's Cancer Group AML-D24, we will stratify patients with newly diagnosed ML-DS according to age at diagnosis, GATA1 mutation status and treatment response. Patients <24 months of age at the diagnosis with a GATA1 mutation who achieve morphological complete remission and are negative for flow cytometric measurable residual disease at the end of induction will be classified as the low-risk (LR) group and receive less intensive chemotherapy. The other participants, stratified into the non-LR group, will be treated with AZA-combined chemotherapy. The objective of this trial is to evaluate the efficacy and safety of AZA-combined chemotherapy in children with non-LR ML-DS. The primary endpoint is the 3-year event-free survival in the non-LR group. The planned enrolment is 145 patients, including 47 non-LR patients. The study period consists of a 7-year accrual period (including an enrolment suspension period for interim analysis) followed by a 3-year follow-up period for a total duration of 10 years. ETHICS AND DISSEMINATION: This study was approved by the National Hospital Organization Review Board for Clinical Trials (Nagoya, Japan) on 12 May 2025 and was registered in the Japan Registry of Clinical Trials (https://jrct.mhlw.go.jp/latest-detail/jRCTs041250031). Written informed consent will be obtained from all patients and/or their guardians. The results of this study will be disseminated through publication in peer-reviewed journals and presentation at national and international scientific conferences. TRIAL REGISTRATION NUMBER: jRCTs041250031.
