Introduction: The essential oil of Shikuwasa (Citrus depressa Hayata) primarily contains limonene and γ-terpinene, which have potential applications in stress management and relaxation. However, the psychological or physiological relaxation effects of Shikuwasa essential oil on humans are still unknown. This study aims to investigate the short-term relaxation effects of Shikuwasa essential oil, one of the less-studied varieties, compared to inhaling odour-free air in young female adults. Methods: and analysis: This study is a two-arm, parallel-group, open-label, randomised controlled superiority trial. Forty young female adults will be assigned with a 1:1 allocation ratio to either the Shikuwasa essential oil inhalation group or the odour-free air inhalation group. The primary outcome measure will be subjective tense arousal (subscale of the Japanese version of the University of Wales Institute of Science and Technology Mood Adjective Checklist). Secondary outcomes include objective measures: miosis rate and peripheral skin temperature for evaluating autonomic nervous activity, and cerebral blood flow (assessed using near-infrared spectroscopy) for evaluating central nervous activity. Since these objective outcome measures cannot be performed at the same time, we divide our experiment into three phases and participants will inhale sample vials for 2 min in each experiment. We will also evaluate individual preferences/impressions regarding inhaled samples and any adverse events. Ethics and dissemination: The study protocol has been reviewed and approved by the Research Ethics Committee of the Faculty of Medicine, University of Miyazaki (reference no: I-0074). The findings of this study will be disseminated to academic and professional audiences via publications in peer-reviewed journals and presentations at academic conferences, and to the broader public via public talks and media/press releases. All study findings, whether negative or positive, will be reported. Trial registration: UMIN Clinical Trials Registry (UMIN-CTR), UMIN000053914. Prospectively registered on March 20, 2024.

Excessive carbohydrate intake increases the risk of obesity and type 2 diabetes and worsens glycemic control in diabetic patients. We conducted a randomized, single-blind, placebo-controlled trial in 19 healthy adults to compare the glycolipid metabolism effects of eating low-carbohydrate cake (carbohydrate content except erythritol and fiber: <5 g per 100 g) to those from eating regular-carbohydrate cake. Although there was no significant differences in the primary endpoint of blood glucose levels at 60 minutes after eating, the secondary endpoints of blood glucose and insulin levels at 30 minutes after eating, incremental area under the curve, and maximum concentration were all significantly lower in the low-carbohydrate cake group than in the regular-carbohydrate cake group. The active glucagon-like peptide-1 (GLP-1) level was significantly higher in the low-carbohydrate cake group than in the regular-carbohydrate cake group at 60 minutes postprandially, and free fatty acids were higher in the low-carbohydrate cake group than in the regular-carbohydrate cake group at 90 minutes postprandially but were not markedly increased compared with the preprandial levels. There were no marked differences in the triglyceride, remnant-like particle cholesterol (RLP-C), or ApoB-48 levels. In conclusion, intake of low-carbohydrate cake suppressed postprandial blood glucose elevation and increased the concentration of active GLP-1 in healthy subjects. The health effects of long-term consumption of low-carbohydrate cake remain to be determined.

© 2020 The Author(s). Background: Adult T-cell leukemia/lymphoma (ATLL) is a peripheral T-cell malignancy caused by human T-cell leukemia virus type 1. The clinical course of ATLL is very heterogeneous, and many organs, including the gastrointestinal (GI) tract, can be involved. However, there are few detailed reports on ATLL infiltration in the GI tract. We investigated the clinical characteristics of ATLL infiltration in the GI tract. Methods: This retrospective observational single-center study included 40 consecutive ATLL patients who underwent GI endoscopy. The patients' demographic and clinical characteristics and endoscopic findings were analyzed retrospectively. Patients with ATLL who were diagnosed by histological examination were divided into two groups based on GI tract infiltration. Results: Multivariate analysis revealed that the absence of skin lesions was significantly associated with GI infiltration (P < 0.05). Furthermore, the infiltration group tended to have similar macroscopic lesions in the upper and lower GI tracts, such as diffuse type, tumor-forming type, and giant-fold type. Conclusions: GI endoscopy may be considered for ATLL patients without skin lesions.

Objectives To clarify the dissemination status of cognitive behavioural therapy (CBT) in Japan under the national health insurance scheme.
Design Retrospective observational study.
Setting National Database of Health Insurance Claims and Specific Health Checkups of Japan.
Participants Patients who received CBT under the national health insurance scheme from fiscal years (FY) 2010 to 2015.
Primary and secondary outcome measures We estimated the change rate and the standardised claim ratio (SCR) for the number of patients receiving CBT and analysed the association between the CBT status and several regional factors.
Results We found that (a) a total of 60 304 patients received CBT during the study period; (b) the number of patients receiving CBT was highest in the first year (−1.8% from FY2010 to FY2015); (c) the number of patients who received CBT per 100 000 population decreased (or remained at zero) in most prefectures (32 out of 47); (d) there was a maximum 424.7-fold difference between prefectures in the standardised claim ratio for CBT and (e) the number of registered CBT institutions was significantly associated with the number of patients who received CBT.
Conclusions The provision of CBT did not increase in the first 6 years (FY2010–2015) after its coverage in Japan’s national health insurance scheme. Further studies including a questionnaire survey of registered CBT institutions are required to get more detailed information on the dissemination of CBT in Japan.
