A 17–year–old female patient presented with acute abdomen and ileus, and based on the findings of abdominal CT, she was diagnosed as having a left paraduodenal hernia with jejunal clustering beneath the inferior mesenteric vein. She gave a previous history of two episodes of intestinal obstruction of unknown cause, with the condition improving spontaneously with just observation on both occasions. For the present condition, she underwent successful elective laparoscopic release of the jejunal invagination and direct closure of the hernia orifice. However, on POD7, she developed signs of bowel obstruction with vomiting and abdominal pain, and CT and a gastrointestinal series revealed limited jejunal stenosis with inflammatory edema. We diagnosed the patient as having postoperative bowel stenosis with inflammatory edema caused by the surgery. Therefore, in accordance with the treatment for panniculitis, we carefully elected to treat the patient with a steroid administered by intravenous injection, which resulted in prompt and dramatic recovery of the jejunal obstruction and the patient recovered and was discharged. Laparoscopic repair is a simple and useful procedure for left paraduodenal hernia, and we showed the usefulness of steroid therapy for postoperative inflammatory jejunal stenosis.

A 14-year-old boy was seen at our hospital because of right lower quadrant abdominal pain and high fever. Abdominal contrast-enhanced CT scan demonstrated large quantities of ascites, swelling of the appendix, and well-thickened sigmoid colonic wall with nodular calcification. Emergency operation was performed with a suspected diagnosis of generalized peritonitis caused by perforated appendicitis. Based on intraoperative findings, there were large volumes of serous ascites, and numerous disseminated nodules were found in the whole abdominal cavity. The main sigmoid colon tumor had invaded the pelvic wall diffusely. The appendix had swollen, but there were no perforations. We performed appendectomy and transverse colostomy. The histopathological diagnosis was mucinous adenocarcinoma-signet ring cell carcinoma. Despite 14 courses of adjuvant chemotherapy with FOLFOX + bevacizumab and three courses with FOLFIRI + bevacizumab, the patient died of peritonitis carcinomatosa 350 days after the operation. Colon cancer extremely rarely affects children. To date, only 29 cases have been reported in Japan since 1983. We consider that colon cancer should not be excluded from differential diagnoses even in children.

A 41-year-old female met with a traffic accident and was taken to our hospital, Miyazaki Prefectural Nichinan Hospital. She was bruised on the chest and the abdomen, and complained of severe abdominal pain. Although computed tomography (CT) examination revealed no internal organ damage in the peritoneal cavity, the abdomen had a platelike hardness, and traumatic peritonitis was strongly suspected. While we planned the exploratory laparotomy, we found she had a history of familial Mediterranean fever (FMF). We diagnosed the abdominal pain as being caused by FMF. She was treated conservatively and the abdominal pain eased quickly. FMF is a disease that is characterized by periodic fever and serositis (pleuritis / peritonitis), developing paroxysmally due to factors such as stress. In this case, we could distingnish FMF from traumatic peritonitis accurately, and unnecessary exploratory laparotomy was avoided.

A 75-year-old man who suffered from dysuria with elevated serum prostate specific antigen (PSA) level was diagnosed as having prostate carcinoma by needle biopsy. Upon further examination, Type 2 rectal carcinoma was found in the anterior wall of the lower rectum concurrently. MRI imaging of the pelvis revealed that the prostate carcinoma was adjacent to the anterior wall of the rectum above the anal canal and also close to the rectal carcinoma. CT revealed multiple lymph node metastases of mesorectal, lateral and inguinal lesions and PET-CT showed metastases to some bones. After one cycle of anti-androgen therapy for Stage D prostate carcinoma, the PSA level was normalized and shrinkage of the metastatic lateral lymph node (LLN) was assessed, followed by laparoscopic abdominoperineal resection for the rectal carcinoma. Pathological findings of the rectal cancer showed well-differentiated adenocarcinoma, pT2, pN0, pStage I and the origin of the mesorectal lymph node (MLN) metastasis was prostate carcinoma with neuroendocrine differentiation. Metastatic prostate carcinoma to the MLN is very rare but it is important to keep this unsuspected pattern of lymphatic spread in mind, especially in case of infiltrative high-risk prostate carcinoma.
