A 41-year-old female met with a traffic accident and was taken to our hospital, Miyazaki Prefectural Nichinan Hospital. She was bruised on the chest and the abdomen, and complained of severe abdominal pain. Although computed tomography (CT) examination revealed no internal organ damage in the peritoneal cavity, the abdomen had a platelike hardness, and traumatic peritonitis was strongly suspected. While we planned the exploratory laparotomy, we found she had a history of familial Mediterranean fever (FMF). We diagnosed the abdominal pain as being caused by FMF. She was treated conservatively and the abdominal pain eased quickly. FMF is a disease that is characterized by periodic fever and serositis (pleuritis / peritonitis), developing paroxysmally due to factors such as stress. In this case, we could distingnish FMF from traumatic peritonitis accurately, and unnecessary exploratory laparotomy was avoided.

A 75-year-old man who suffered from dysuria with elevated serum prostate specific antigen (PSA) level was diagnosed as having prostate carcinoma by needle biopsy. Upon further examination, Type 2 rectal carcinoma was found in the anterior wall of the lower rectum concurrently. MRI imaging of the pelvis revealed that the prostate carcinoma was adjacent to the anterior wall of the rectum above the anal canal and also close to the rectal carcinoma. CT revealed multiple lymph node metastases of mesorectal, lateral and inguinal lesions and PET-CT showed metastases to some bones. After one cycle of anti-androgen therapy for Stage D prostate carcinoma, the PSA level was normalized and shrinkage of the metastatic lateral lymph node (LLN) was assessed, followed by laparoscopic abdominoperineal resection for the rectal carcinoma. Pathological findings of the rectal cancer showed well-differentiated adenocarcinoma, pT2, pN0, pStage I and the origin of the mesorectal lymph node (MLN) metastasis was prostate carcinoma with neuroendocrine differentiation. Metastatic prostate carcinoma to the MLN is very rare but it is important to keep this unsuspected pattern of lymphatic spread in mind, especially in case of infiltrative high-risk prostate carcinoma.

A 67-year-old female, previously treated for arterial fibrillation with warfarin, was transferred to our hospital because of sudden abdominal pain, nausea, and vomiting. A CT scan showed a superior mesenteric artery (SMA) obstruction and small bowel ischemia. She was treated with transcatheter arterial injection of urokinase, and abdominal exploration. Twenty cm of necrotic small intestine was resected and the color and motility of the remnant intestine was intact. Seven days after the first operation, she complained of abdominal pain and fever. No ischemic changes of the remnant small intestinal ischemia were detected on CT imaging, but a small intestinal enema revealed mucosal necrosis. We decided to carry out a second exploration. The remnant intestine exhibited good pulse, color, and motility, but its circumference was unclear and we detected a stricture during intraoperative endoscopy. The resection line was detected using endoscopy, followed by repeat resection, to obtain as short a remnant as possible. Based on the histopathological findings, the patient was diagnosed as having an ischemic stricture of the small intestine. We report herein on a case of ischemic stricture of the small intestine after SMA thrombosis. A small intestinal enema was useful for diagnosis, and we determined the resection line with intraoperative endoscopy.

Surgical Indications in 22 Patients with Hepatic Portal Venous Gas

The subjects were 22 patients with hepatic portal venous gas (HPVG) findings who were treated at our hospital. We compared Group A, in which surgery was ultimately indicated (including preoperative deaths), with Group B, which received successful conservative treatment. We then examined the surgical indications for patients with HPVG in the emergency room. CT findings such as extrahepatic portal venous gas (85.7% vs. 25.0%, <i>P</i>=0.0083), pneumatosis intestinalis (64.3% vs. 12.5%, <i>P</i>=0.0310), and ascites (78.6% vs. 25.0%, <i>P</i>=0.0260) were significantly more common in Group A. The appearance of extrahepatic portal venous gas, pneumatosis intestinalis, and ascites on CT findings might be important objective indicators of a need for surgery in patients with HPVG.
