We treated an 81-year-old man with distal bile duct cancer with pancreaticoduodenectomy following preoperative arterial modification for celiac axis (CA) obstruction. Contrast-enhanced CT revealed increased wall thickness of the distal bile duct, indicating the cancer and CA obstruction. The arterial arcade at the pancreatic head and dorsal pancreatic artery (DPA) were well developed and were the main blood supply to the hepatic artery. Angiographic balloon occlusion of the inferior pancreaticoduodenal artery (IPDA) revealed good blood flow to the hepatic artery via the DPA, so the IPDA was embolized with coils to modify arterial blood flow. Nine days thereafter, pylorus-preserving pancreaticoduodenectomy with D2 lymph node dissection was performed, and curative resection with cancer-free surgical margin was obtained histologically. The DPA was preserved, and hepatic arterial flow observed intraoperatively was maintained. No events related to poor blood flow occurred perioperatively. The many varieties of CA and superior mesenteric arteries must be kept in mind.

Two Cases of Abdominal Stab Wounds Caused by Self-harm and Fighting

Case 1: A 65-year-old man was transported to our hospital by medical helicopter after stabbing himself in the abdomen with a kitchen knife. The stabbing had occurred on the day prior to the request for emergency help. The kitchen knife had been removed, a large quantity of intestinal tract had exited the wound, and some clothing had attached to the intestinal tract. Although an enhanced CT examination showed no signs of visceral injury, we performed an emergency operation to return the intestinal tract to the abdominal cavity. No visceral injuries were seen, and the intestinal tract was easily returned to the abdominal cavity using an isolation bag. Case 2: A 43-year-old man who was stabbed in the abdomen with a kitchen knife during a fight with another person was transported to our hospital. Upon arrival, the kitchen knife had not been removed from the wound, and the patient's vital signs were stable. An enhanced CT examination showed that the kitchen knife was located in the vicinity of the pancreas and stomach, and we performed an emergency operation. Although the kitchen knife had passed through the greater omentum and reached the omental bursa, no visceral injuries were seen. While abdominal stab wounds are relatively rare, surgeons must be able to respond quickly and know how to treat such injuries.
