On 1 January 2024, a large earthquake occurred in Japan’s Noto region. Many buildings collapsed as a result of violent shaking. Electricity and water supplies were cut off, and communications were disrupted. On 5 January, four days after the earthquake, we visited Noto and conducted disaster-relief activities. This report integrates and discusses the results of the site visits, information broadcasts by public institutions, and previous research. Evacuation centers lacked water and proper sanitation, leading to health issues, including infectious diseases. Disaster Medical Assistance Teams (DMAT) were delayed in implementing infection control measures. Isolated evacuation centers faced communication and supply challenges. Infrastructure restoration, power supply, and toilet facilities at evacuation centers were delayed because of geographical challenges. It is important to have a team that can determine and carry out the necessary activities on site, even without instructions from the DMAT. It is believed to be effective to decide in advance how volunteer teams and the private sector will conduct their activities, assuming that they will be unable to contact public institutions during a disaster. In large-scale disasters, evacuees must operate evacuation centers autonomously. To achieve this, it is necessary for residents to regularly come together as a community. Systematically recording and accumulating these experiences will contribute to improved disaster prevention and mitigation planning. We hope that the experiences we obtained through the abovementioned disaster will be useful for preparing for future disasters.

BACKGROUND/OBJECTIVES: There are concerns about the adverse health effects of behavioral changes linked to coronavirus disease 2019 (COVID-19). We conducted a survey to investigate changes in lifestyle habits, including exercise and eating, during the COVID-19 pandemic and their association with changes in weight. SUBJECTS/METHODS: We conducted a survey of 5,000 people in Hakui City, Japan, to assess their lifestyles and diets during the COVID-19 pandemic. A total of 3,992 complete responses were received. We also obtained pre-and post-pandemic health check-up data for 704 of the respondents. These health data were combined with the results of the questionnaire survey to identify factors associated with weight changes. Data were analyzed for 704 individuals. RESULTS: The mean body mass index was 22.5 ± 3.1 kg/m2 for respondents whose weight did not change and 25.1 ± 2.7 kg/m2 for respondents whose weight increased. The pre-pandemic mean values for abdominal circumference in females and for gamma-glutamyl transferase in males tended to be higher in those whose weight increased. Those with decreased weight tended to be older. Respondents who were already overweight were more likely to gain weight because of COVID-19-related changes in their lifestyle. In males, alcohol consumption was directly associated with weight, and in females, abdominal circumference was more important. CONCLUSION: The study found pre-pandemic overweight individuals likely gained more weight during it, with alcohol consumption being a significant factor for males. Weight loss was more crucial for those over 70, rather than weight gain.

Background: The impact of chronic kidney disease (CKD) on healthy life expectancy and healthcare costs requires research. This study examined associations between CKD and healthy life expectancy, and its economic burden. Methods: This study of community-dwelling adults residing in Hakui City, Ishikawa Prefecture, Japan used data from the National Health Insurance database between 2012 and 2022. Participants were grouped by baseline estimated glomerular filtration rate (eGFR) (< 45, ≥ 45 to < 60, ≥60 to < 75, ≥75 to < 90, and ≥ 90 mL/min/1.73 m²). The primary endpoint was a composite of becoming a care level ≥ 2 or death. Multivariable Cox proportional hazards models were used to calculate the risk regarding time to the primary endpoint. Secondary endpoints were the annual medical and long-term care costs. Results: The 5,592 participants had a mean follow-up of 6.4 years. The hazard ratio was 1.86 (95% confidence interval [CI]: 1.35 to 2.55) for the eGFR < 45 group and 1.60 (95% CI: 1.13 to 2.25) for the eGFR ≥ 90 group, both compared with the eGFR ≥ 60 to < 75 group. Both annual costs were significantly higher in the lower eGFR groups than in the higher eGFR groups. For the eGFR < 45 group, the median medical care cost was 0.38 million yen/year in all participants and the median long-term care cost was 0.40 million yen/year in primary endpoint achievers. A lower eGFR was correlated with longer unhealthy years of life. Conclusions: Higher and lower eGFRs were associated with increased risks of reduced healthy life expectancy. A lower eGFR was associated with higher medical and long-term care costs.

<b>Introduction:</b> This study evaluated the detection of monthly human mobility clusters and characteristics of cluster areas before the coronavirus disease 2019 (COVID-19) outbreak using spatial epidemiological methods, namely, spatial scan statistics and geographic information systems (GIS).<b>Methods:</b> The research area covers approximately 10.3 km<sup>2</sup>, with a population of about 350,000 people. Analysis was conducted using open data, with the exception of one dataset. Human mobility and population data were used on a 1-km mesh scale, and business location data were used to examine the area characteristics. Data from January to December 2019 were utilized to detect human mobility clusters before the COVID-19 pandemic. Spatial scan statistics were performed using SaTScan to calculate relative risk (RR). The detected clusters and other data were visualized in QGIS to explore the features of the cluster areas.<b>Results:</b> Spatial scan statistics identified 33 clusters. The detailed analysis focused on clusters with an RR exceeding 1.5. Meshes with an RR over 1.5 included one with clusters for 1 year which is identified in all months of the year, one with clusters for 9 months, three with clusters for 6 months, three with clusters for 3 months, and four with clusters for 1 month. September had the highest number of clusters (eight), followed by April and November (seven each). The remaining months had five or six clusters. Characteristically, the cluster areas included the vicinity of railway stations, densely populated business areas, ball game fields, and large-scale construction sites.<b>Conclusions:</b> Statistical analysis of human mobility clusters using open data and open-source tools is crucial for the advancement of evidence-based policymaking based on scientific facts, not only for novel infectious diseases but also for existing ones, such as influenza.

<文献概要>はじめに 私たちは、日本地域看護学会次世代育成推進委員会(以下、当委員会)のメンバーです。当委員会は、次世代の地域看護を担う人を応援する活動をするために設置されました。社会の変化は著しく、地域看護への期待や役割は一層増していると思います。昨年度、日本地域看護学会(以下、当学会)では、2040(令和22)年に向けて地域看護を推進すべく「日本地域看護学会定義に基づく2040リサーチアジェンダ24」(以下、2040アジェンダ)を作成しました。次世代の地域看護を発展させていくためには、研究や実践を担う若手人材の育成が不可欠になります。そこで若手を中心とした委員で当委員会を構成し、戦略を練りました。10月には"YouTubeはじめました"という企画を実施し、12月よりLINEオープンチャット「地域看護の次世代を考えるグループ」を開設しました。本稿では当委員会のこれまでの活動と今後についてご報告いたします。なお、当委員会の活動は、当学会ホームページでご覧いただけます(http://jachn.umin.jp/committee12.html)。

すでに一般に公表している、要支援者がスマホに自己情報を入力し災害へ備えるためのアプリ(K-DiPS Solo)からのSOSを受信、支援者からの安否確認、かつ要支援者の情報を用いて自動でBCPシート、優先対応者リスト、スタッフリスト作成機能を実装したK-DiPS BCPを開発した。K-DiPS BCPを地域に実装することで、要支援者の自助を助け、支援者との連携により個々の実情に合った共助体制に寄与する。本稿では、K-DiPS BCPを概説する。(著者抄録)

<b>目的:</b>高知県を対象に地理情報システムを用いて,訪問看護ステーション(VNS)のサービス提供体制別にみた空間アクセシビリティの評価を目的とする.<b>方法:</b>オープンデータを用いてVNSのサービス提供体制の分布を求めた.VNSの空間分布と自動車の法定速度で,30分で到達可能な勢力範囲を割り出した.<b>結果:</b>最寄りのVNSまでの直線距離の平均は2,199.8 m,最短が623.1 m,最長が6,793.5 mだった.全てのVNSの勢力範囲とサービス提供体制別の勢力範囲の比較では,24時間対応,緊急対応,精神看護で一部空白区域があったが,概ね30分の勢力範囲をカバーしていた.リハビリ専門職が配置されたVNSの勢力範囲において,西部の沿岸部に空白区域が認められた.<b>結論:</b>VNSの偏在による空間アクセシビリティの不公平性に対して,人口やVNSの報酬体系,移動距離,地理的特徴を考慮したソリューションの検討が必要である.

Following the Great East Japan Earthquake, vertical evacuation shelters (VES) were constructed to reduce tsunami risk. Childcare centers (CCs) in the inundation area are required to evacuate to the nearest VES in the event of a tsunami. The study aim was to identify CCs and VES predicted to be inundated by a Nankai Trough earthquake-generated tsunami, and to clarify CC inundation risk. We identified 52 (45.6%) CCs in the tsunami inundation area and found that 14 (25.9%) would evacuate toward the tsunami. If the walking speed was 2.24 km/h and a 0.3 m tsunami arrived in 10 min, nine (17.3%) CCs would be late to safe evacuation. If the tsunami arrival time was 20 min, four (7.7%) CCs would have late evacuation. At a walking speed of 1.00 km/h, 38 (73.1%) and 20 (38.5%) CCs would have late evacuation, with tsunami arrival times of 10 min and 20 min, respectively. Evacuation direction is important in avoiding tsunami damage. An evacuation strategy is needed that evacuates people away from the tsunami, and takes into account children’s age, walking speed, and evacuation method. The evaluation of tsunami risk in this study may support the development of tsunami countermeasures in other coastal areas with latent tsunami risks.
