© 2019 Background: Elbow injuries are common in young baseball players and evaluating the characteristics of young baseball players in a large-scale population is necessary. However, few studies have compared playing position, length of baseball experience, physical findings, and elbow pain in young baseball players. This retrospective multi-regional cohort study sought to document the physical findings at the elbow in Japanese elementary school baseball players and to examine the relationship between playing position, length of baseball experience, physical findings at the elbow, and elbow pain. Methods: In 2014, 720 fifth-grade baseball players from four regions in Japan were invited to participate in a questionnaire survey and undergo physical examination to obtain data on position played, length of baseball experience, presence of elbow pain, and physical findings at the elbow, including range of motion, tenderness, and valgus stress test results. Potential risk factors associated with elbow pain and correlations between physical findings and playing position, length of baseball experience, and elbow pain were investigated. Results: The 720 subjects had a mean age of 10.4 years and 29.4% reported having experienced elbow pain for ≥1 week. Risk factors for elbow pain included playing pitcher and catcher, playing pitcher and fielder, and length of baseball experience. The most frequent finding was limitation of flexion (21.1%) followed by tenderness of the medial epicondyle (18.3%) and a positive valgus stress test (14.3%). These three findings were also significantly associated with elbow pain. Elbow extension was limited in 14.9% of subjects but there was no correlation with length of baseball experience or elbow pain. Conclusions: Our data show that elementary school baseball players who experienced elbow pain are likely to have valgus stress overload. Elbow pain might be prevented by not playing pitcher and catcher.

【はじめに】我々は2016年4月よりPatient Specific Instrumentaion(PSI)を使用しTKAを施行している.その正確性をCTにて解析した.【対象】2016年4月から2018年8月までにPSIを使用したTKAを施行した34例40膝中,術後CTによる評価を行えた15例16膝.手術時平均年齢は69.9歳,原因疾患は全例内側型変形性膝関節症であった.【方法】術前後に全下肢CTを撮影.bone matching法で術前計画との誤差を測定し,目標設置角度より3°以上の誤差をoutlierとし評価した.術後成績はJOA scoreにて評価した.【結果】平均手術時間は104分,大腿骨側の内外反設置角度の正確性は0°±2.4°,outlier 19%(3/16),脛骨内外反設置角度0.8°±2.5°,outlier 38%(6/16),脛骨後傾設置角度1.3°±3.5°,outlier 50%(8/16)であった.JOA scoreは術前58.1±6.8点から術後80.3±7.2点に有意に改善した.【考察】大腿骨・脛骨側内外反・後傾ともに高率にoutlierを認めた.cutting block設置や特有のボーンソー使用法に関してlearning curveの存在があると考える.

【はじめに】非常に稀な特発性脊髄硬膜下血腫を経験したので報告する.【症例】78歳女性.突然の背部痛と呼吸困難感が出現し,前医に救急搬送された.MRIで胸椎レベルに脊髄硬膜外血腫を疑う所見があり,徐々に膝立て困難となったため,当院に転院搬送となった.当院初診時MMT4まで改善していたが,その後もMMT2まで動揺する下肢麻痺を認めた.再検MRIでTh4-L2の脊髄腹側に血腫があり,さらにT12/L1のT2高輝度変化を認めた.受傷後2週目の脊髄造影検査で,褐色の髄液を認め,特発性硬膜下血腫と診断した.保存加療にて血腫は自然消退,独歩可能となった.【考察】特発性硬膜下血腫の画像上の特徴はMRIで三日月型とされているが,その他の出血性病変と鑑別困難なこともあり,稀ではあるが脊髄硬膜下血腫を念頭に置いて診療することが必要である.

【はじめに】母趾以外の外傷性Metatarsophalangeal(以下,MTP)関節脱臼は稀とされ,大部分は徒手整復可能であるが不能例も存在する.今回,観血的整復を要したMTP関節脱臼の一例を経験したため報告する.【症例】18歳男性,バイク運転中に軽自動車へ衝突し受傷,当院搬送.Xp,CTにて第3~5趾MTP関節の背側脱臼と第2中足骨頚部骨折を認めた.透視下に徒手整復を施行し,第5趾のみ整復できたが,3~4趾は不能であった.同日,観血的整復へ移行し,第3・4趾間MTP関節に陥頓した関節包,虫様筋を認めた.整復阻害因子と考えられ,背側進入を選択し,陥頓の解除で容易に整復可能であった.4週MTP関節をこえるシーネ固定,踵部荷重を許可し,12週後の観察時,再脱臼は認めていない.【考察】観血的整復を要した第3・4・5外傷性MTP関節脱臼の一例を経験した.徒手整復不能例は速やかに観血的整復への移行が必要である.整復困難であった3・4足趾には関節内に陥頓した関節包及び虫様筋を認め整復阻害因子と思われた.

種子骨が嵌頓し整復阻害因子となった母趾趾節間関節脱臼の陳旧例の1例

【はじめに】種子骨の背側陥入を認める母趾趾節間(IP)関節脱臼は稀である.今回4週経過した陳旧性のIP関節脱臼を経験したため,報告する.【症例】40歳男性.28日前,2mの高さから転落し左足をついて受傷.近医にて左母趾の裂創の処置を受けるも,疼痛は改善せず当院受診となった.単純X線にて母趾IP関節の脱臼を認め,CTでIP関節の背側に種子骨を認めた.徒手整復は不可能であった.同日背側アプローチで進入し,背側の種子骨と掌側板を底側に押し込むと脱臼を整復できたが,易脱臼性を認めIP関節固定と背側block pin施行した.術後は6週でPin抜去し,再脱臼なく経過し,最終JSSF score(6か月)は95点であった.【考察】IP関節脱臼で種子骨の背側嵌頓を認める場合,観血的整復が必要であり,本症例でも同様であった.

© 2013 IEEE. Loss of meniscal function due to symptomatic meniscal tears or meniscectomy leads to biomechanical instability and articular cartilage degeneration. Synthetic meniscal implants ought to ideally restore normal joint contact mechanics and thus forfending the overlying cartilage from degeneration. The purpose of this study was to quantify the contact stresses in both tibiofemoral compartments and joint kinematics during a gait cycle after implantation of a synthetic meniscal implant. Anatomically-detailed finite element model of the knee joint was developed from magnetic resonance images of a healthy female volunteer. Gait analysis was conducted using a three-dimensional motion capture system and computed the knee joint forces and moments and quadriceps muscle forces for a complete walking cycle. The effects of a synthetic meniscal implant on joint mechanics during gait were studied by conducting finite element simulations for the following meniscus conditions: (i) intact meniscus, (ii) meniscus with complete radial posterior root tear, (iii) total meniscectomy, (iv) isotropic meniscal implant, and (v) shell-core composite meniscal implant. Posterior root tear and total meniscectomy caused substantially increased contact stresses in both tibiofemoral compartments and altered tibial kinematics. Compared to posterior root tear and total meniscectomy, the isotropic and composite meniscal implants reduced the peak contact stresses in both compartments and reduced the cartilage nodes with higher contact stresses by disseminating the load over a large surface area. The shell-core composite meniscal implant resulted in lower contact stresses in the medial compartment relative to the other meniscus conditions. This study demonstrated that posterior root tear and total meniscectomy leads to detrimental changes in joint mechanics. Superseding the injured meniscus with a synthetic meniscal implant restored the joint mechanics close to the intact meniscal state. This novel synthetic meniscal implantation approach appears to be a promising strategy for treating patients with severe meniscal injuries.

© 2019 Juvenile Paget disease (JPD1), an autosomal-recessive disorder, is characterized by extremely rapid bone turnover due to osteoprotegerin deficiency. Its extra-skeletal manifestations, such as hypertension and heart failure, suggest a pathogenesis with shared skeletal and cardiovascular system components. In spite of this, the effects of anti-hypertensive drugs on bone morphometry remain unknown. We administered an angiotensin II type 1 receptor blocker, olmesartan (5 mg/kg/day) to 8-week-old male mice lacking the osteoprotegerin gene, with and without 1 μg/kg/min of angiotensin II infusion for 14 days. Olmesartan treatment decreased systolic blood pressure, and echocardiography showed increased left ventricular systolic contractility. Three-dimensional micro-computed tomography scans demonstrated that olmesartan treatment increased trabecular bone volume (sham, +176%; angiotensin II infusion, +335%), mineral density (sham, +150%; angiotensin II infusion, +313%), and trabecular number (sham, +407%; angiotensin II infusion, +622%) in the tibia. Olmesartan increased cortical mineral density (sham, +19%; angiotensin II infusion, +24%), decreased the cortical bone section area (sham, −16%; angiotensin II infusion, −18%), decreased thickness (sham, −18%; angiotensin II infusion, −31%), and decreased the lacunar area (sham, −41%; angiotensin II infusion, −27%) in the tibia. Similar trend was observed in the femur. Moreover, olmesartan decreased angiotensin II-induced increases in tartrate-resistant acid phosphatase concentrations in plasma, but it affected neither type I procollagen N-terminal propeptides, nor the receptor activator of nuclear factor kappa-B ligand. Our data suggest that blockade of the angiotensin II type 1 receptor improves bone vulnerability, and helps to maintain the heart's structural integrity in osteoprotegerin-deficient mice.
