Purpose: No studies have used stress analysis with finite element analysis (FEA) to determine the causes of and mechanisms underlying rotator cuff tears. Therefore, we performed a biomechanical evaluation of the changes in stress distribution on the rotator cuff using three-dimensional (3-D) FEA. Methods: The 3-D FEA model of shoulder joint allowed for abduction angles of 0°, 45° and 90° from the plane of the scapula and included the anatomical insertion points of the three major rotator cuff tendons and the middle fibres of the deltoid muscle. Stress distribution of the supraspinatus tendon on 3-D FEA was validated by a comparison with cadaveric and two-dimensional finite element model. Results: The principal stress peaked in the region approximately 1 cm proximal to the insertion of the supraspinatus tendon. Furthermore, the stress on the joint side increased at the anterior edge of the supraspinatus tendon at abduction angles of 45° and 90°. Conclusion: There are differences in stress changes between the joint side and bursal side of the supraspinatus tendon within the angles of abduction. The maximal tensile stress was observed on the articular side of the anterior edge of the supraspinatus tendon at 90° abduction. Our results indicate that the difference in tensile stress between the two layers results in delamination and causes partial-thickness tears. As Japan has become a super-aging society a new focus has been placed on locomotive syndrome (Locomo), musculoskeletal ambulation disorder symptom complex (MADS), frailty and sarcopenia. Locomo is a condition whereby long-term care or support is needed or may be needed in the future due to weakness of the locomotive organs. MADS is a condition presenting with unstable balance or walking difficulty, which leads to a high risk of the patient falling down or giving up excursions outside the home. In a broad sense, MADS patients also belong in the Locomo category. The prevention and treatment of Locomo and MADS are very important, because these conditions typically result in deterioration of the exercise function and loss of mental and physical health. The aim of Locomotive syndrome exercises is to reduce pain, and to restore and improve joint function. We need to take a comprehensive approach to Locomotive syndrome, including lifestyle modification, muscle exercise, stretching and therapeutic exercise. Two basic, simple and commonly used exercises are single-leg standing and squats exercises.

Although it is not easy to differentiate neuropathic scoliosis caused by neural axis abnormalities (NAAs) from idiopathic scoliosis for lack of neurological signs, neuropathic scoliosis caused by NAAs has some specific clinical findings. The purpose of this study was to investigate the characteristics of neuropathic scoliosis caused by NAAs and to discuss the essential points of diagnosis and treatment. From 1998 through 2008, 622 new patients visited our scoliosis outpatient clinic. Of those, 12 patients with neuropathic scoliosis caused by NAAs diagnosed by cervical MRI were involved in this study. They included 5 boys and 7 girls with a mean age of 10 years and 5 months. We investigated the relationship of NAAs, treatment methods for scoliosis, and effects of neurosurgical treatment for prognosis of outcome. There were 10 cases of syringomyelia with Chiari malformation type I, 1 case of syringomyelia alone, and 1case of spinal cord tumor. Surgery was performed in 3 cases, an under arm brace was prescribed in 6 cases, and observation was selected in 3 cases. Scoliotic curves were improved in 4 out of the 11 cases on whom neurosurgical intervention for the relationship of NAAs were performed. The Cobb angles before surgery of these 4 cases were less than 30° and 3 of the 4 cases were 8 years old or less. We would emphasize that early treatment for the relationship of NAAs at younger age (≤8 yrs) and before curve progression (<30°) should be actively enforced for favorable prognosis of neuropathic scoliosis caused by NAAs. © Kuroki et al.; Licensee Bentham Open.

To compare the efficacy of 2-octylcyanoacrylate (Dermabond; Ethicon Inc) with standard suture repair in the management of cervical laminoplasty. Fourty-five patients were enrolled in this study. Twenty-two patients were using Dermabond, and 23 were repaired using standard suture. All patients underwent surgery in 2010 and were observed three months or more after operation. Operation time, prone position time, incision length, intraoperative blood loss, days of drain and gauze removal and times to replace the gauze were examined. There were no adverse events in both groups. Significant difference in operation and prone position time, intraoperative blood loss, days of drain removal were not observed. The average days required for gauze removal using Dermabond was 6.73 days, which was significantly less than that required for suture repair (11.87 days) (p<0.05). And the average number of times to replace the gauze using Dermabond was 3.86, which was significantly less than that required for suture repair (5.22 times) (p<0.05). In the management of cervical laminoplasty, Dermabond is an effective repair technique.
