The purpose of this study was to evaluate knee muscle strength following anterior cruciate ligament (ACL) reconstruction in female. We treated 12 female patients with a bone-patellar tendon-bone autograft (BTB) and 29 female patients with a semitendinosus-gracilis autograft (STG) in ACL reconstruction. Knee extensor and flexor strength were measured by KIN-COM before operation, and 3 months, 6 months and 9-12 months after operation. The angular velocity was set at 60 and 180 degrees/second. The results showed that the STG group was better than BTB group on extensor and flexor muscle strength after 3 and 6 months, but there were no significant differences after 9 to 12 months. BTB group patients over 30 years of age showed the worst recovery of extensor muscle strength after 9 to 12 months. The results suggest a delay or deficit in the recovery of knee muscle strength in middle-aged female with BTB in ACL reconstruction.

In Japan, sports medicine has developed significantly in conjunction with goals to improve athletic levels. Moreover, Japan has become an aging society where promoting health and preventing disease through sports have become commonplace. However, due to the participation by people of all ages in many sports activities, the occurrence of low back pain has increased. Low back pain is classified into several types, and the absence of adequate treatment will have a significant impact on prognosis. Therefore, early and adequate diagnosis of low back pain is very important. When low back pain is caused by sports, the specificity of each sport event should be noted. This paper describes the lumbar sport injuries and disorders such as sprains, fractures of the lumbar vertebrae, disc herniation, spondylolysis, and vertebral end-plate disorder during the growth period. In sports disorders, precise diagnosis and adequate treatment are necessary, and it is essential to avoid continuing treatment aimlessly when symptoms persist.

A Case of Abnormality of the Bilateral Medial Meniscus and Lateral Meniscus

We report a case of abnormality of the bilateral lateral meniscus and medial meniscus. A ten-year-old boy complained of pain and locking of the right knee. On magnetic resonance imaging of the right knee, there was a longitudinal tear on the posterior portion of the lateral meniscus. In arthroscopic findings, the medial meniscus had abnormality of attachment on the anterior portion, and the lateral meniscus comprised of two parts which were completly separeted and duplicated at the midportion. The anterior part was connected to the capsule and the posterior part to the cartilage surface of the tibia plateau at the midportion. We peformed meniscorrhaphy for longitudinal tear on the posterior portion.<br>We checked his left knee for bilateral congental abnormalities. In the arthroscopic findings of the left knee, the shape of the lateral meniscus was almost the same as the right knee, and the medial meniscus had two layers on the anterior portion (double-layers medial meniscus). We found a horizontal tear on the posterior portion, and peformed partial menisectomy on it.<br>We could not find literature on this abnormality of the lateral meniscus and double-layers medial meniscus.

We performed osteochondral grafting with mosaicplasty on a patient diagnosed with ankle sprain in spite of osteochondral lesion of the talar dome. A 14-year-old boy sprained his left ankle during a volleuball practice game when he landed on the floor after spiking the ball and his ankle was forced to a varus position. He was diagnosed with ankle sprain and only prescribed compresses. But because of persistent ankle pain at sports, he consulted our hospital six months after injury. The X-ray films showed osteochondral lesion (Berndt and Harty's classification Stage III ) at the antero-lateral part of the talus. Although we performed a preliminary arthroscopic survey of the ankle and osteochondralsynthesis (PDS pin) for the lesion in combination with autograft transplantation six months after injury, it resulted in nonuinon. We performed osteochondral graft with mosaicplasty on the lesion two years after the first operation and gained good results after the re-operation. When traumatic osteochondral lesion of the talus is diagnosed, it is important to differentiate it from ankle sprain. Osteochondral graft with mosaicplasty seems useful in salvage treatment for osteochondral lesion.

A Case Report: Cauda Equina Syndrome as a Postoperative Complication of Lumbar Disc Herniation

We experienced a case of cauda equina syndrome as a postoperative complication of lumbar disc herniation. The patient was a 57-year-old man and suffered from left leg pain and lumbago. We diagnosed lumbar disc herniation at L4-5 and L5-6 and performed L4-5 and L5-L6 discectomy and posterolateral fusion. Three days after operation, he did not complain of leg pain and lumbago, but he experienced numbness in the perineum and urinary retention. We carried out emergency MR imaging study, but no compressive findings were demonstrated. The patient was expected to recover spontaneously, and numbness in the perineum and urinary retention were resolved three months postoperatively. Vascular insufficiency of the cauda equina was suspected.

Hypertrophic pachymeningitis is a rare disorder that causes intracranial or spinal thickening of the dura mater. We report three cases of hypertrophic pachymeningitis. All these patients presented symptoms such as back pain, paraparesis, and bladder-bowel disturbance. The cases of hypertrophic pachymeningitis include infections such as tuberculosis, fungal and meningococcal meningitis, and autoimmune diseases such as rheumatoid arthritis, Wegener's granulomatosis, and sarcoidosis.<br>Serologic test results for RA factor were positive in two patients, and no arthritis was found. One patient was diagnosed as meningitis, which was considered to be a field disease. MRI scan revealed marked dural thickening with circumference. It was hypointense on both T1 and T2WI, and contrast enhancement was prominent. All patients were successively cured by steroid therapy. The patients showed initially good response to steroid therapy, but in one patient, symptoms recurred when the steroid dosage was reduced.<br>Laminectomy and markedly thickened dura compression were performed, and the cord was noted. This was partially excised for the purpose of curing and diagnosis. Echo proved useful for the diagnosis of hypertrophic pachymeningitis.
