© 2019 The Japanese Orthopaedic Association Background: This study aimed to estimate the prevalence of locomotive syndrome (LS) risk in age groups using the LS risk test proposed by the Japanese Orthopaedic Association. Based on this, this study identified the lifestyle-related signs that lead to LS, by clarifying the association between LS risk and lifestyle factors. Methods: A total of 728 individuals aged 20–91 years who were without specific mobility disorders were evaluated. Participants were asked to perform the LS risk test and complete a self-administered original questionnaire survey for this study. The Mann–Whitney U test was used to examine the relationships of sex and age with LS. The prevalence of LS by sex and age was compared using the Chi-squared test or Student's t-test, as appropriate. The Mantel–Haenszel test was used to examine the relationship of each component of the lifestyle factors. Results: Overall, 36.4% of males and 49.1% of females were classified into the LS group; the prevalence of LS was significantly higher in females than males (p < .001). The prevalence of LS tended to increase with age in both males and females (p < .05). As for health condition, pain in the locomotive organs, knees, back, and hip joints was significantly associated with LS (p < .05). Of the lifestyle-related diseases, only hypertension (p < .05) was significantly associated with LS, and drug treatment was significantly associated with LS (p < .05). As for lifestyle, anxiety about physical fitness, self-assessed poor health, irregular dietary habits, and lack of exercise were also associated with LS (p < .05). Conclusions: Interventions to prevent LS are important during its early stages, especially for females, and lifestyle-related signs of LS, such as pain in the locomotive organs, lifestyle-related disease, drug treatment, and a lower subjective evaluation of health must not be overlooked.

© 2019 The Author(s). Background: Bone tunnel enlargement following primary anterior cruciate ligament (ACL) reconstruction with soft tissue graft might be a severe disadvantage for revision surgery. The postoperative rehabilitation protocol including the non-weight-bearing periods were different depending on the surgeon or institute. To determine the relationship between femoral bone tunnel enlargement and the postoperative non-weight-bearing period after double-bundle ACL reconstruction with hamstring grafts. Methods: Forty-two patients who underwent primary double-bundle ACL reconstruction with hamstring grafts were divided into two postoperative non-weight-bearing protocol groups: 1-week non-weight-bearing postoperatively (group A, n = 19); and 2-week non-weight-bearing (group B, n = 18). Five cases were excluded due to additional knee injury, pregnancy, and lost to follow-up. Bone tunnel enlargement was evaluated by computed digital radiographs (anteroposterior (A-P) and lateral views) taken on the first postoperative day and at 12 months. Each tunnel diameter was shown as a percentage to the maximum joint width of the proximal tibia in the A-P view, or a percentage of the maximum diameter of the patella in the lateral view. To determine the incidence of tunnel enlargement, percentage diameter changes of more than 10% were defined as an enlarged tunnel. The magnitude of tunnel enlargement and the standard clinical evaluation were also evaluated. Results: There were no significant differences between groups in the incidences of anteromedial and posterolateral bone tunnel enlargement, both in the A-P and lateral views (2 × 2 Chi-squared test). The magnitude of femoral posterolateral bone tunnel enlargement was significantly greater in group A in the A-P view (p = 0.01) and lateral view (p = 0.03) (Mann Whitney U-test). Twelve months after surgery, the Lysholm score and Tegner activity level scale were not significantly different between the groups. Conclusions: This prospective, clinical and radiographical study showed that early weight-bearing protocol after double-bundle ACL reconstruction with hamstring grafts might have the potential risk of significant postoperative femoral bone tunnel enlargement of the posterolateral bundle. There was no significant difference in clinical outcomes by postoperative non-weight-bearing period. To reduce and prevent the femoral bone tunnel enlargement, the comprehensive management could be considered and required to establish the suitable early stage rehabilitation protocol after surgery. 箸を用いた実際の食事に必要とされる正常な全身の関節角度と,その角度変化を検討することを目的に,三次元(3D)動作解析を行った.健常者45名の食事動作を,モーションキャプチャーシステムにて測定し,上下肢・頸部・体幹の関節角度を明らかにした.このうち,肘関節屈曲が最大約133°および最小約99°,手関節背屈が最大約30°および最小約10°であることを示した.また,関節角度の経時変化から,箸で食物を挟む時期に最も肩関節が内旋し,食物を口へ取り込む時期に最も肩関節が屈曲と外転し,頸部が伸展・右側屈・右回旋することがわかった.これらは,われわれが対象者の食事を評価し介入するための基礎知識になると考えられた.

BACKGROUND: Patients undergoing total or partial arthroscopic meniscectomy for treating traumatic meniscal tears are at greater risk of developing knee osteoarthritis (OA) due to increased mechanical load. The purpose of this study was to evaluate the effects of a valgus unloader brace in the medial meniscectomized knee joint during the gait cycle. METHODS: A three-dimensional finite element model of the knee joint was developed using the substructures segmented from magnetic resonance images. Experimentally measured forces and moments for one complete gait cycle, without brace and with brace at three different alignment angles (0°, 4°, and 8°), were applied to the finite element model, and the changes in the tibiofemoral contact mechanics were estimated. RESULTS: The brace in 0°/4°/8° valgus alignment modes reduced the total contact force in the medial compartment by 16%/46%/82% at opposite toe off and 18%/17%/29% at opposite initial contact events, while it increased the total contact force in the lateral compartment by 31%/81%/110% at opposite toe off and 30%/38%/45% at opposite initial contact events, respectively, when compared to the unbraced meniscectomized knee. CONCLUSIONS: Increasing the valgus alignment from 0° to 4° and 8° resulted in a greater reduction of contact conditions (total contact force, total contact area, peak contact pressure) in the medial compartment and vice versa in the lateral compartment. This decrease in contact conditions in the medial compartment infers enhanced knee joint function due to a valgus unloader brace, which translates to increased knee-related confidence. Results suggest choosing a higher valgus alignment angle could potentially increase the risk for the onset of osteoarthritis in the lateral compartment, and this computational model could be used in validating the effectiveness of braces on joint health.

© 2019 Quantification of baseball hitting mechanics under game conditions help players to become successful batters and prevent injuries. Inertial measurement units (IMUs) can measure motion without any spatial restriction and are thus becoming a popular tool to investigate sports biomechanics. Biomechanical analysis of hitting requires the accurate detection of key events including “foot-off” while leaning back (F Off ), “foot-on” during forward swing (F On ), and ball impact. Ten male university baseball players hit a ball suspended on a T pole five times in kick-hitting and glide-hitting styles. Three IMUs were attached on mid-pelvis and on each hand to record acceleration and orientation data. The key events identified by the three IMUs were compared with those retrieved by an optical motion capture system with force platforms. The timings of the local peak acceleration of the pelvis in the direction of the pitcher that were recorded by the IMU closely matched those of F Off and F On events detected by the ground reaction force. Root mean square error (RMSE) between each measurement for the F Off and F On events were 0.024 and 0.031 s, respectively. The timing of the negative peak of acceleration in the proximal direction of the hands corresponded to the impact time determined by an optical motion capture system. RMSEs for the knob and barrel-side hand were 0.009 and 0.011 s, respectively. Our results demonstrate how IMUs can be useful for analyzing baseball hitting mechanics.
